  • Monitors policies and standards for the medical aspect of the patient care program.
  • Provides medical direction to hospice patients on routine and emergency basis.
  • Acts as a liaison between the patient’s attending physician and the IDG members.
  • Attends IDG meetings and participates in the on-going development of individualized care plans.
  • Certifies/re-certifies patient’s Medicare hospice benefit eligibility.
  • Participates in the development, coordination and implementation of hospice policies and procedures.
  • Acts as consultant to patient’s primary physician.
  • Presents educational in-services for IDG and the community.
  • Facilitates liaison activities between Hospice and local medical community.
