- Functions as the Case Manager – provides coordination of care and ensures continuous assessment of each of the patient’s and family’s needs and implementation of the interdisciplinary plan of care.
- An RN completes the initial assessment within 48 hrs of admission along with the IPOC.
- An RN also completes a comprehensive assessment within 5 days after the Election of Benefits (EOB) along with the CPOC.
- The nursing update of the comprehensive assessment will be completed at least every 15 days and the information from the assessment will be used to update the CPOC.
- Provides supervision to LPNs and Hospice Aides
- Provides dietary counseling
- Ensures that the nursing needs of the patients are met as identified in the patient’s initial assessment, comprehensive assessment and updated assessments.
